We help you prepare for appointments, put your preferences and questions into words, and understand the options in front of you. This can include joining appointments virtually or in person. Our aim is to help you feel confident and heard through serious illness or end-of-life care.
02.Care Coordination
We help you organize appointments, providers, medications and follow-up care so everyone involved stays on the same page. You get steady, compassionate support that fits your situation. You can share your needs through the intake request below.
03.Medical Record Organization and Review Support
We organize and review your medical records so you can better understand your health care information. We note questions and topics to bring to your medical team and give you a clear summary to help you prepare for those conversations.
Fees are based on each client's individual needs and scope of services and are discussed before services begin.
